By cube testing at an accredited laboratory at 7 and 28 days. You should receive a copy of the result for every pour — that is a basic requirement, not a luxury.
It depends on the number of floors, the spans, the foundation system and the price of steel and cement at the time. Which is why any price quoted without drawings is a guess.
